What we need from you:

Please fax the following information to

  • face sheet
  • history and physical
  • results of consultations (include notes from PM&R if applicable)
  • diagnostics
  • therapists' and social workers' initial evaluations
  • therapy notes (past two days)
  • labs and vitals (past 72 hours)
  • current medications
  • most recent progress notes from providers
  • nursing notes (past two days)
  • imaging

For patients with a spinal cord injury, please fax the ASIA scale, if available. After we review the records, you will receive a call from one of our team members to discuss the results of the assessment and help facilitate admission, if appropriate.

    Inpatients Program

    At Pamax Biotech´s campus, inpatient rehabilitation care is provided in the 24-bed inpatient Comprehensive Rehabilitation Unit. 

    Care in the unit is designed to maximize your independence and assist with your transition back into the community.

    Those admitted to the unit are people who can realistically expect to improve functional performance within a predictable time period. You're expected to be able to participate in three hours of therapy a day. Therapies provided in the inpatient setting include physical, occupational, speech and recreational therapy.

    Inpatient care programs include rehabilitation for:

    • Spinal cord trauma
    • Brain injury
    • Stroke
    • Neurological disorders
    • Major multiple trauma
    • Complex medical disorders
    • Bones (orthopedic)
    • Pediatric rehabilitation and physical medicine needs

    Outpatient Program

    Pamax Biotech, provides a wide range of inpatient and outpatient rehabilitation and community services.

    Our goal is to partner with you to help you achieve your highest level of independence, health and wellness. We do this by providing excellent services, innovative programs, ground-breaking research and barrier-shattering advocacy.

    Our expert rehabilitation team works with children and adults who have injuries and disabilities of all kinds.

    Our providers include physicians and clinical staff who specialize in physical medicine and rehabilitation. We value you and will work with you, your family and care team to support you as you work to reach your goals. Our goal is that you are able to become as independent as possible to do what is most important to you in your life. Whatever that may be.
